Sensory Impairment team

The Sensory Impairment team works to assess and deliver services to improve quality of life and maximise independence in the lives of sensory-impaired Hammersmith & Fulham residents aged 18 and older.

Community Independence team

The Community Independence team provides support to Hammersmith & Fulham residents aged 18 and older, to help people regain independence after an illness, hospital discharge, or other situations that have impacted their ability to perform daily tasks.

The team works with individuals to set goals, assess needs, and provide practical assistance and equipment to help them live more independently at home for longer.

Occupational Therapy service – Primary Intervention OT Team

Provides holistic assessments and interventions for adults with functional challenges due to physical, psychological, neurological, or age-related conditions.

The service aims to maximise residents' independence, safety, and participation in daily activities at home and in their local community through personalised support and risk reduction. It helps delay or avoid the need for formal care packages and residential placements, and prevents hospital admissions by allowing residents to live safely and independently at home. A strength-based approach ensures inclusion, co-production, and empowerment of both residents and their carers.

Deprivation of Liberty Safeguards team

The DoLS team manages the Deprivation of Liberty Safeguards (DoLS), a part of the Mental Capacity Act 2005. This framework protects vulnerable individuals in care homes and hospitals who lack the capacity to consent to their care, treatment, and accommodation.

Our role is to ensure that any deprivation of liberty is necessary, proportionate, and in the person's best interests.

We assess and authorise these arrangements, appoint representatives to advocate for the individual. We provide the right to challenge the deprivation in the Court of Protection.

Adult Social Care Front Door - Promoting wellbeing and independence for adults

Adult Social Care Operations promotes wellbeing and independence for adults from age 18, residing in Hammersmith & Fulham, who require care and support and for carers whose needs we have a duty to meet, whilst also seeking to ensure that adults at risk are safeguarded from harm.

We are committed to providing high-quality information, advice and guidance and where required care and support that respects an individual's right to choice and to live a life that makes sense to them.

We use 'strengths-based practice' and ensure the adult is at the centre of everything we do. We work collaboratively with partner organisations, both internal and external, as co-produce and develop services. The Adult Social Care Front Door meets the needs of our residents in H&F who may require our services.

Mental Health

Our vision within integrated health and social care is focussed on implementing new ways of working in personalisation and a strength-based approach to enable individuals to live as independently as possible to make informed choices and control over their lives and the care that they receive.

We have a robust framework of supervision for social workers that ensures professional development wherever you are in your career. We provide an excellent post-qualifying training programme, including a sound, supportive framework for AMHP training. Qualified AMHPs will be joining a well-established centralised AMHP service.

Transitions team

The Transitions team use creative community-based planning, a wealth of knowledge in Special Educational Needs and Disabilities in Children's and Adult services, Health (including CHC funding), Care Leavers, Housing and Direct Payments and strong multiagency partnership to support young people and their families in effectively planning for adulthood, ensuring that they are supported to navigate the often complex route into Adult Services.

Learning Disabilities team

Our Learning Disabilities Social Service Team provides person-centred support and services around daily living, housing, employment, and community access to maximise independence and improve the quality of Hammersmith & Fulham residents aged 18+ with learning disabilities.
