- Personalised plan and timetable
We create a tailored weekly schedule with clear goals and activities.

Begin with either a structured start date or a gradual transition plan with visits and introductions.
Transitioning into the service
We understand that starting a new service can be a big step. That's why we offer a personalised transition process, starting from age 17, to help individuals feel safe, confident, and supported.
Each person is assigned a dedicated keyworker who works closely with families and professionals. Transition plans may include:
- Visits to current services or home
- Introductions to ACAT staff
- Gradual familiarisation with the new environment
Our goal is to build trust and ensure a smooth, respectful transition into ACAT.
