Family hub referrals

Guidance for professionals on referring families to family hubs, for support with accessing services.

Family hubs can provide support and signposting to H&F residents, so they can access the services that they need. With consent from the parent/carer or young person, family hubs can share their story with other services.

To avoid duplication between services, this service is only available to residents who are not already receiving this support from another service.

Areas of need may include:

  • parenting support
  • family relationships and domestic abuse
  • midwifery and health visiting
  • debt and housing
  • school placement
  • youth services
  • special educational needs and disabilities
  • social isolation
  • mental health and wellbeing
  • child safeguarding.

The referral process

Referrals can be made by:

  • professionals, through the family hub referral form
  • residents themselves, usually when they register with family hubs

Referrals are screened by Family Navigators and a Family Hub Manager to ensure:

  • no other services are working with the resident
  • the referral is suitable for family hubs to pick up

If the referral is appropriate

If the referral is appropriate, the Family Navigator will contact the resident to discuss their needs and plan next steps. They will then arrange a meeting at a family hub location, if possible.

If the referral is not appropriate

If the referral is not appropriate, it will be returned to the referrer with an explanation. For example, because it may be more appropriate as a GP referral.

Complex cases

High-need cases may be taken to the Family Hubs Multidisciplinary Meeting (FHMD), to ensure efficient and integrated wrap-around support is provided.

A referral may be made to the H&F Initial Contact and Advice Team (ICAT) to assess. ICAT may allocate the case to the Early Help Service.

Make a referral

The referral should include:

  • clear and detailed information about the reason for the referral
  • what support was provided prior to the referral and the outcome
  • questions or concerns
  • resident details and the background history
  • resident consent.

Information for residents who wish to self-refer

If you know a family who would like to refer themselves, you can share the information below with them.

Residents will first need to register with family hubs to access the support available. To do this they can complete the Family Hub Registration Form

They can also visit any of the family hub locations in person to register.

The family hub locations are:

  • Family Hub Old Oak Community Centre

  • Family Hub Stephen Wiltshire Centre (specialist SEND centre)

'School readiness' activities and services for children under 5 years old

Under 5's sessions are delivered by the family hub children's centres and by our partner, the Urban Partnership Group. These sessions are designed and delivered in line with the Early years foundation stage (EYFS) statutory framework (GOV.UK), which supports children's development.

This includes:

  • encouraging children to be independent
  • early communication skills
  • social and emotional skills
  • sharing a snack time
  • circle time at the end of the sessions
  • good routines such as positive sleep routines
  • discussions with parents around toilet training.
