Residents helped to get back on their feet by outstanding H&F team

The H&F Council reablement team has again been rated Outstanding by the Care Quality Commission (CQC).

The Hammersmith & Fulham Council team that helps residents to get back on their feet after a hospital stay or medical treatment has again been rated Outstanding by the country’s Care Quality Commission (CQC), which regulates care nationally.

Our “reablement” team offers up to six weeks free, focused care to rebuild people’s independence and support their mental as well as physical wellbeing. Alongside physical therapies, this may include restoring people’s confidence in going out, travelling on their own or going back to education.

Everyone gets a personalised service according to their needs. The team works 365 days a year from 8am to 10pm and supports more than 2,000 H&F residents a year. Its 43 staff include therapists, accessors, coordinators, business support and carers.

Cllr Ben Coleman, H&F’s Cabinet Member for Health and Adult Social Care, said, “Our reablement team gets lots of compliments from residents but it’s great to get top marks from the regulator for the second time in a row. Given all the extra pressures of Covid, including more early hospital discharge, I’m really chuffed we’ve managed to maintain such a high standard.”

The CQC awarded top marks in every one of five key areas: being safe, effective, caring, responsive to people’s needs and well-led.

The reablement team is very popular with residents. Josephine Addai-Adutwum was delighted with the support she got in 2021 after a hospital stay.

This is the second time in a row the team has been rated Outstanding by the CQC, meaning that their quality of service has remained exceptional over the past four years.

Free home care

Home care has been free since 2014, when H&F abolished all home care charges, the only council to do so.
