A who's who guide to our SEND teams

The range of people who support families with children and young people with special educational needs and disabilities (SEND) locally, the services they offer and how to get in touch with them.

Travel care team

Who we support

Families, schools, short breaks and other settings to identify and risk assess the travel needs of children with special educational needs and disabilities.

Young people with an education, health and care plan can get support until they are 25 if they are still in education.

What we do

We:

  • receive requests for travel support for school and social care
  • complete reviews of travel care plans
  • complete travel care assessments in partnership with the family and professionals involved with the family
  • liaise with transport providers to monitor the quality of their service
  • consider the needs, age, vulnerabilities and skills of the young person and the journey travel is requested for to make an assessment for travel care
  • identify the best means of providing this (if travel support or transport is required) - that might be independent travel training, an individual travel budget, transport, and or escort support
  • complete travel care assessments in partnership with the family and professionals involved with the family
  • seek occupational therapy support for assessments where required
  • address and troubleshoot any problems with transport arrangements
