A who's who guide to our SEND teams

The range of people who support families with children and young people with special educational needs and disabilities (SEND) locally, the services they offer and how to get in touch with them.

Integrated keyworkers

Who we support

This team supports families with a child or children up to the age of 18 with permanent and substantial disabilities and where there are significant complexities and the need for partnership working.

What we do

  • Provide early intervention support for children, young people and families
  • Act as the main point of contact for families, professionals and agencies
  • Coordinate and or attend meetings with professionals and families as required
  • Identify any gaps and ensure appropriate referrals are made
  • Maintain regular proactive contact with families and provide practical and emotional support
  • Undertake family support assessments, team around the family (TAF) meetings and develop, monitor and review plans
  • Offer advice and information to families on SEND services
  • Feed into draft education, health and care plans
  • Ensure that families, children and young people are fully involved in decisions about their care and support
  • Liaise with schools, health and other agencies
