A who's who guide to our SEND teams

The range of people who support families with children and young people with special educational needs and disabilities (SEND) locally, the services they offer and how to get in touch with them.

Short breaks review team

Who we support

Children and young people from birth to their 18th birthday, who are identified as having a permanent and substantial disability and their families.

What we do

We:

  • meet with families to discuss their request for support as the front door to SEND social care support at the Stephen Wiltshire Centre
  • provide advice and support to other professionals about short breaks and other local services
  • help families identify the outcomes they and their child want to achieve
  • take short break requests to panel as required
  • undertake reviews of packages of support as required
  • co-produce assessments with families to identify levels of short break packages
  • agree with families’ support plans which identify which services and support best meet their identified needs
  • work closely with colleagues across SEND services to support families effectively, including referring families to other professionals where required
