Joint Health and Wellbeing Strategy 2016-21

The Health and Wellbeing Board is responsible for improving the health and wellbeing of the Hammersmith & Fulham population and must have a plan that explains how it will do this. This plan, known as the Joint Health and Wellbeing Strategy (JHWS), must be based on evidence of local need found in the local Joint Strategic Needs Assessment (JSNA) and set priorities for action that will make a real impact on people’s lives. The outcomes the board wants to achieve will inform the buying (“commissioning”) of health and social care services that will address needs identified in the JSNA.

Developing the Joint Health and Wellbeing Strategy

The Health and Wellbeing Board hosted a number of conversations throughout 2016 with members of the public, staff from local authority and NHS organisations, and voluntary and community sector representatives and completed a thorough review of the evidence of local need contained in the JSNA. This helped the board identify four priority areas where it agreed to focus efforts over the next five years.

  1. Supporting good mental health for all
  2. Supporting children, young people and families to have the best possible start
  3. Addressing the rising tide of long-term conditions
  4. Building a sustainable health and social care system that is fit for the future
