LSCP practice and serious case reviews

The purpose of a Local Child Safeguarding Practice Review is to:

  • Establish whether there are lessons to be learned from the case about the way in which local professionals and agencies work together to safeguard and promote the welfare of children.
  • Identify clearly what those lessons are, how they will be acted on, and what is expected
  • As a consequence, improve inter-agency working and better safeguard and promote the welfare of children.

We will publish Serious Case Reviews that have been completed by the previous LSCP for Hammersmith & Fulham, Kensington and Chelsea or Westminster and for the new LSCP for Hammersmith & Fulham.


On Monday 19 July 2021 the Local Safeguarding Children Partnership (LSCP) published the ‘Adam’ Serious Case Review

The 'Adam' Serious Case Review was originally commissioned by the Local Safeguarding Children Partnership for Hammersmith & Fulham, Kensington and Chelsea and Westminster City councils. In April 2021, this partnership was disbanded and replaced by two LSCPs: LSCP for Kensington and Chelsea and Westminster; and LSCP for Hammersmith & Fulham.


On Tuesday 27 April 2021 the Local Safeguarding Children Partnership (LSCP) published the ‘David’ Serious Case Review

The 'David' Serious Case Review was originally commissioned by the Local Safeguarding Children Partnership for Hammersmith & Fulham, Kensington and Chelsea and Westminster City councils. In April 2021, this partnership was disbanded and replaced by two LSCPs: LSCP for Kensington and Chelsea and Westminster; and LSCP for Hammersmith & Fulham.
