0-19 Healthy Child Programme

Hammersmith & Fulham Council's Health and Social Care Commissioning Team are in the process of recommissioning 0-19 Healthy Child Programme services. The Healthy Child Programme (HCP) currently consists of two services: health visiting and school nursing. The school nursing service includes transitional support for young people up to the age of 25 with special educational needs and disabilities (SEND).

We want to ensure that any future 0-19 service will continue to contribute to achieving positive healthy outcomes for children and families. The ultimate aim is that all children and young people are developing well and ready for school, ready to learn and able to achieve their potential.

We are consulting

We want to hear views from parents, carers and young people as well as health, social care and education professionals and stakeholders about the Healthy Child Programme services — how they are currently, and what they should look like in the future — to help us better understand and inform the design and delivery of these services going forward.
