Declaration - Data Protection Act 1998
As
part of our funding requirements we are obliged to pass the
information you have given on this form to our funders: London
Borough of Hammersmith and Fulham / the URBAN Partnership
Group. The details will be kept and maintained on a database
which will be used to find out how people have benefited in
terms of training and employment, monitor equality of opportunity,
to derive general statistics, help us derive our own statistics,
and for other purposes described below.
Our
funders will also be kept informed of any further contact
we have. They may sometimes use the details you have provided
to contact you directly. This is in order to confirm that
you have received the service(s) we have informed them about,
and is part of the process of verifying the information we
give them.
If
you go to another provider funded by the relevant party or
parties, all the information you have given will also be made
available to them to help assess your needs better.
If
you register with Intermedia and when you have finished with
the service, you will receive short update requests through
the mail or by telephone for a limited period of time. This
is necessary for our funders to see how you have progressed
after leaving us, so that the effectiveness of our service
can be monitored.
Unless
you have indicated otherwise at the bottom of the form, our
funders will assume you are interested in training or job
opportunities related to your stated employment aims. They
will therefore sometimes pass your mailing details from database
to local projects who have such courses or jobs on offer.
This will last for as long as it is useful to you.
