Q:  How do I apply for a Blue Badge? 

A:  Please call the Permit Services team on 020 8753 5046 to request an application form or for more information. You can bring your application form in or renew your Blue Badge at Hammersmith Town Hall - ground fl  between 2pm - 4.30pm Monday to Friday.
   
You will need to supply the following items with your application:

• 2 recent passport-style photos (or other photos that can be cut to that size).
• Completed and signed photos and application.
