How housing support for older residents can help

Housing support is a free, short term service for residents having difficulties managing their home and staying independent in the community. We have two housing support providers that specifically help older people with these needs. If you are eligible for the service you can choose between the following two providers:

  • Nottinghill Pathways and
  • Sagecare


What can both providers help you with?

Both providers can help you with the following:

  • Managing your money, bills and rent
  • Making your home warm, safe and comfortable
  • Arranging appointments
  • Filling in forms and writing letters
  • Referring you to other services you may need
  • Connecting you with your local community


Who can get the service?

The service is available to Hammersmith and Fulham residents aged 55 and over, living in any type of accommodation in the borough and includes:

  • Council tenants
  • Private tenants
  • Sheltered housing tenants
  • Housing association tenants
  • Home owners


How can I get a service?

To apply, please complete a pdf version referral form which can be posted or WORD version referral form which can be emailed to  housing.support@lbhf.gov.uk or by telephoning the Housing Support Service on 020 8753 1437. You do not have to fill in the form yourself. The form can be filled in by someone else on your behalf.


How long will I wait before I hear from you?

We will contact you within five working days of receiving your form to let you know if you are eligible or not.  

If you are not eligible we will advise you of any other services available that might be of interest or help.
If you are eligible you will be asked to choose between the two providers. Once you have chosen the provider, your details will be passed to them and they will arrange to meet with you.
