Use this Referral Form to refer cases to the LBHF Housing Welfare Panel (HWP)

The purpose of the HWP is to consider and award priority to applicants who have circumstances, which may not always be appropriate to present to the Council’s Medical Advisor.  Cases for the HWP will only be considered where there are needs, as outlined in the Housing Welfare Panel Procedure and the welfare of the applicant and/or their household is being adversely affected by their current housing situation.  Referrers must evidence what solutions they have sought prior to referral to the Housing Welfare Panel, as detailed in the Referral form

Referrals must be presented to the panel by support agency, primary care trust, children services, registered social landlord professionals involved in the case.

Who cannot refer to the HWP?

• Legal representatives
• Housing applicants

cannot refer, self-refer nor attend any panel.  Following presentation, the referrer will be notified in writing of the outcome of the panel.

The HWP meets on a monthly basis.  All paperwork must be submitted within the document submission guidelines and any paperwork presented outside this framework may not be considered.

Please ensure you complete this form fully and in all the fields provided.  Incomplete forms will be returned and will result in the case not being considered by the Panel.
