Supporting Your Choice.(Personalisation)
The Supporting Your Choice Project has now ended and the new assessement and review processes is live for residents contacting adult social care services. This means that the 6 steps to getting adult social care (pdf, Dec 2010, 203kB) easyread version ( pdf, 901kB) is now used for all assessments and reviews for residents
Supporting Your Choice was H&F's name for Self-Directed Support (Personalisation), giving to residents entitled to care services choice and control over how their allocated budgets are spent and who provides their services in order that they can live their own lives independently and to the full.
