Options day service

This service helps people develop skills to continue living at home and also helps people access a range of services and community facilities. 

Services include: 

  • assessment
  • care
  • supervision and training
  • support to participate in the community
  • help finding out about mainstream services
  • support in using leisure activities     

People who use the service are encouraged to plan for their immediate and future needs and have a key worker to help them with this.   

We work with people who live with a learning disability and additional support needs. This may include: 

  • physical disability
  • frail health
  • mental healh problems
  • autistic spectrum disorder
  • dementia
  • sensory impairments
  • challenging behaviour     

People can use this service for up to five days a week or on a planned basis for specific sessions or activities.

Who is it for?

Options Day Service supports adults with learning disabilities aged 18 years and over, (including those with high complex care needs or high challenging needs) over who are FACS eligible. 

FACS stands for ‘Fair Access to Care Services’. This is the guide produced by the Department of Health 2003, telling us how we can decide who is eligible for adult social care. There are four levels of eligibility - critical, substantial, moderate and low.  In Hammersmith & Fulham we provide services for people who have ‘critical, substantial and greater moderate needs’.  For more information about who can have services, call 0845 313 3935.

What services are offered?

Options currently offer three areas of service: 

  • From the Goldhawk Rd base both community and building based activates are provided for residents with high support and complex care needs. 
  • From the Blomfontein Rd base, the MAPS Project provides individually tailored opportunities for those with highly challenging support needs.
  • The Enterprise Project supports access to community leisure, education and training with a focus on increasing independence. Help is also available to find and support work placements.
