Community access team - helping people to achieve independence in the community

Community access team

The community access team provides a high quality, community-based, flexible support programme for adults with physical disabilities. We are a support service that enables you to access and positively engage in community-based activity and maintain or increase your independence in the community

Referrals

An initial referral will be made to us by a social worker or other professional. We will then arrange to visit you at home, assess your needs and agree on how you want us to support you to achieve your goals and help you agree a twelve week tailor-made action plan

Some possible areas for skill development and support include:

  • Identifying goals and objectives
  • Confidence building 
  • Accessing further education and employment 
  • Travel training to use buses, the tube and taxis via our accredited travel training programme 
  • Support to access and use shops, cafes and restaurants

Key principles of the service:

  • Being community rather than centre-based
  • Being time-limited – 12 weeks in the first instance
  • Being person-centred so that each adult has his or her own tailor-made plan
  • Referrals to the service are made by social workers or other relevant professionals
