Sunberry Independent Living Service
The service helps people achieve maximum independence and control over their lives through choosing and then receiving the right level of intervention and support. We also assist people with physical disabilities to identify what services they need to maintain or increase their independence in the community. Independence doesn’t only mean doing things on one’s own, it also means doing things with the assistance of others on one’s own terms.
The Sunberry Independent Living Service has a number or working partnerships with education, employment, health and the independent sector that promote social inclusion. These partnerships include such organisations as Ealing/Hammersmith/and West London College, Hammersmith Adult Education, Expert Patient Programme, Community Assessment and Rehabilitation Team and more. We also have an employment project in conjunction with Ealing, Hammersmith and West London College and also work with Disability West and Shaw Trust.
The key principles of the service are:
- developing these types of partnerships
- being community rather than centre based
- being time limited (maximum 1-2 years)
- and, most importantly, being person centered so that each person has his or her own tailor-made plan.
Referrals to the service are made by social workers or other relevant profesionals.
