Lost or stolen freedom passes
Since 1 July 2005 we have made a £12 administrative charge for the replacement of lost or stolen disabled person's passes. There will not be a charge for replacing passes returned to us because they are faulty.
When reporting a lost or stolen pass you need to:
1. Contact your local police station to report the loss or theft of your pass. The police will issue you with a lost property number or a crime report. Keep this safe as you will need it when you come for your replacement.
2. Once you have been to the police please call The Blue Badge and Freedom Pass Team immediately on 020 8753 5046 to advise us of the loss of your card or call in to our office at Room 31, Ground Floor, Hammersmith Town Hall, Kings Street W6 9JU. We will inform Transport for London so that they can cancel the pass. This will take up to 10 working days.
3. We will then let you know when you can return to the Blue Badge and Freedom Pass Team to collect a letter to take to the cashiers' office at the town hall. You will be asked to pay £12 and will be given a receipt to bring back to us. You also need to have the following with you:
- your lost property number or crime report from the police
- 2 recent photographs (signed on the back)
- proof that you live in the borough (e.g. recent gas, electricity or phone bill, current community charge, rent or benefits book, recent letter from your doctor)
You must apply for your replacement pass in person.
