Registering a still birth

A still birth must be registered within three months. To register a still birth you will need a medical certificate, Form 34.

The still birth must have occurred in the London Borough of Hammersmith & Fulham district.

It must be registered by a qualified informant who can give all the information required to register -

• The Mother;
• The Father, where the child's parents were married to each other at the time of
• the still birth or conception;
• If parents are not married then they should attend together.
• The occupier of the house in which the still birth occurred;
• A person present at the still birth;
• The person in charge of the still birth;
• The person who found the still birth.
