Performance and stage licences for school age children
The law states that you need to give 21 days notice for a licence to be issued. This may not always be practical, however due to the limitations of resources in this borough, please give at least seven working days notice for any application to be successful; all information should be included as listed below. Please ensure that the dates and place of performance are stated on the application form, and that all questions are answered. We will not accept “ TBA” against any question.
Further time is needed for complicated film or theatre schedules and also if you are sending in a number of applications at the same time. This time is required so that the licence and child can be checked properly, paperwork completed and a copy has time to reach the borough in which the venue is situated.
Your application must include:
- 2 current passport size photographs
- Copy of the birth certificate unless one is already on file
- Medical certificate (lasts 6months) or parent declaration (if child is already known to us.)
- Permission from the child’s school unless: a) within school holidays b) complete production takes place on non-school days c) if the child is under 5 years old and not yet in full time education
- Telephone & Fax numbers of licensee
- Name & address where the performance is taking place
- Dates and details of performances
- Name and full address of Matron (normally called a chaperone) or name of parent
- Signature and address of parent
Please note that a licence will not be issued if any of the above information is missing or not received by the education department within the above time scale; requests for “open licences” for six months cannot be accepted at the moment.
Licences can be sent recorded delivery, by courier or delivered personally to Cambridge House, Cambridge Grove, Hammersmith W6 0LE at least seven working days in advance of the first rehearsal / performance date. If the reception is closed then envelopes may be left at the front gate of Cambridge House.
