HIV/Aids

A team of social workers (sometimes called care managers) who provide services for people living with HIV and/or AIDS, their carers, partners, families and friends.

These services may include:

  • assessment of social care needs
  • support and care to promote quality of life and maximise independence
  • respite care
  • terminal care
  • advice and information
  • A community worker attached to the team undertakes sexual health promotion work.       

The community nurse specialist provides assistance and support in the community to people taking anti-retroviral medication, dealing especially with adherence issues. Also provides nursing assessments and support and referrals to other organisations.
