Care Coordination

The Care Co-ordination Team identifies and cares for vulnerable adults with long-term conditions who are at risk of frequent hospital admission or A&E attendance.   We offer intense support over short period and look at physical, emotional and social needs.   Our aim is to reduce unplanned emergency admissions, improve quality of life and prevent premature death from these conditions. 

We are a partnership between the Primary Care Trust and social services and our team is made up of care co-ordinators from both nursing and social work backgrounds.
